Bremsende muskelarbeid før dykk kan påvirke risiko for dykkersyke

Eksentrisk trening som gir mikrorupturer i skjelettmuskulaturen halvannen time før et dykk, gjør ikke at det under dykket dannes flere gassbobler i blodårene til rotter. Slike gassbobler kan gi dykkersyke. Dersom det først skulle oppstå mange gassbobler under dykket, tyder våre resultater likevel på at sjansen for å overleve er lavere dersom man har trent før dykket. Dermed kan det tenkes at eksentrisk trening med stor grad av bremsende muskelarbeid, initierer er ukjent prosess som øker risikoen for dykkersyke i samspill med gassbobler. Samtidig overlevde totalt sett like mange rotter et simulert dykk, uavhengig av om de hadde trent før dykket eller ikke. Det kan tyde på at risikoøkningen ved eksentrisk trening kompenseres av andre, gunstige mekanismer som også aktiveres ved trening, for eksempel en økning i nitrogenoksid som utvider blodårene.

Studien inkluderer 42 rotter, som enten løp halvannen time i nedoverbakke eller ikke trente. Å løpe nedover gjør at bremsemekanismen til musklene aktiveres, noe som gir stor eksentrisk belastning og større skader på muskulaturen enn konsentrisk trening. Og vi så betydelig økt muskelskade blant rottene som hadde løpt nedover. Likevel oppstod det altså ikke flere gassbobler i blodårene til rotter som gjennomførte et simulert dykk i trykkammer etter treningen, sammenlignet med rotter som gjennomførte samme dykk uten å ha løpt i nedoverbakke først.

Intervalltrening styrker aldrende hjerter

Jo eldre man blir, jo stivere blir hjertet og dårligere til å fylles med blod mellom hvert slag. Men menn i 70-årene kan trene seg opp til omtrent like god hjertefunksjon som inaktive menn i 20-årene. 12 uker med 4x4-intervalltrening tre dager i uka gjorde at fylningsevnen til hjertet forbedret seg markant hos de tidligere inaktive eldre mennene.

Studien viser også at eldre toppidrettsutøvere har hjerter som pumper bedre enn friske, inaktive eldre, men likevel ikke fullt så godt som hos inaktive yngre. Fylningsevnen til hjertet var derimot minst like god hos de eldre idrettsutøverne som hos de yngre som ikke trente.

Diabetes kan være mest risikabelt for kvinnehjerter

Det er generelt sett langt flere menn enn kvinner som dør av hjerteinfarkt. Blant de som har diabetes er kjønnsforskjellen derimot nesten utjevnet, viser resultater fra Helseundersøkelsen i Nord-Trøndelag.

Nesten 5000 av de 48 000 deltakerne døde av hjerteinfarkt eller ustabil angina i løpet av de 18 årene vi fulgte dem. Risikoen for å dø av slik hjertesykdom var høyest for personer med kjent diabetes og nest høyest for de som fikk påvist diabetes under helseundersøkelsen. For kvinnene med diabetes var risikoen økt med 150 % sammenlignet med hos kvinner med normalt blodsukker, mens risikoøkningen ikke var på mer enn 80 % for menn med diabetes.

Én av seks friske har svekket blodårefunksjon

Blodåre-illustrasjon

16 % av friske kvinner og 17 % av friske menn har svekket blodårefunksjon, ifølge målingene vi gjorde av 4739 deltakere i HUNT3 Fitness-studien. For menn begynner blodårene å svekkes jevnt allerede fra 30-årsalderen, mens nedgangen skyter fart først rundt 45 års alder for kvinner.

Flow-mediert dilatasjon (FMD) målt med ultralyd er et godt mål på endotelfunksjonen, altså blodårenes evne til å utvide seg ved anstrengelse. Svekket blodårefunksjon indikerer tidlig åreforkalkning og er koblet til økt risiko for framtidig hjerte- og karsykdom. Tidligere har det ikke fantes noen referanse for hvilke FMD-verdier man kan forvente å finne hos friske kvinner og menn i ulike aldre, og det store datamaterialet i denne studien bidrar med nyttig info i så måte.

Økning i hvilepuls kobles til tidlig død

Hvis hvilepulsen har økt mye over en tiårsperiode, er risikoen for å dø av hjerte- og karsykdom de neste 12 årene vesentlig høyere enn hvis hvilepulsen har holdt seg stabilt lav. Det kan vi si etter å ha undersøkt endringer i hvilepuls hos nesten 30 000 nordtrøndere som deltok i to utgaver av Helseundersøkelsen i Nord-Trøndelag.

Studien ble publisert i 2011 i svært anerkjente Journal of the American Medical Association (JAMA). Risikoen for å dø av hjerteinfarkt var nesten dobbelt så høy hos de som hadde opplevd en økning i hvilepuls fra under 70 til over 85 slag i minuttet, sammenlignet med de som hadde hvilepuls under 70 ved begge helseundersøkelsene.

Intervalltrening er svært effektivt for hjertepasienter

4x4-intervaller er mer effektivt for kondisjonen etter et hjerteinfart enn standardoppfølging ved norske sykehus, og kan også bidra til at pasientene fortsetter å trene på egen hånd. Studien våre forskere publiserte i 2011 viste høyere maksimalt oksygenopptak i intervalltreningsgruppa også mer enn to år etter at den organiserte treningsperioden på 12 uker var avsluttet.

Til sammen inkluderte studien 107 hjertepasienter som ble tilfeldig fordelt til intervalltrening eller moderat trening i grupper på sykehuset. 69 deltakere ble med på oppfølgingsingstestene etter 30 måneder. Over åtte av ti pasienter i intervallgruppa oppgav at de fortsatt trente minst to ganger i uka, men dette kun gjaldt litt over halvparten av pasientene som hadde fått standardoppfølging.

Fysisk aktivitet kan forutsi kondisjon i framtida

Selvrapportert fysisk aktivitet kan foruti det maksimale oksygenopptaket hos friske voksne over lengre tid. Vi fant en klar sammenheng mellom fysisk aktivitetsnivå på 1980-tallet og faktisk målte kondisjonsnivåer 23 år senere.

Studien tyder på at det er en pålitelig sammenheng mellom selvrapportert fysisk aktivitet og kondisjon. 1843 friske kvinner og menn deltok i studien. De fikk målt kondisjonen sin i forbindelse med Kondisprosjektet i Den tredje Helseundersøkelsen i Nord-Trøndelag, og hadde også svart på spørsmål om egne aktivitetsvaner i den første utgaven av helseundersøkelsen midt på 1980-tallet. Både deltakere som rapporterte å være aktive over tid, og de som hadde økt aktivitetsnivået sitt fra første til siste undersøkelse hadde bedre kondisjon enn de som rapporterte å være inaktive på begge tidspunktene.

Kondisjon avslører hjerte- og karrisiko

Gjennomsnittskondisjonen til aktive kvinner og menn i 50-årene er like høy som kondisjonen til inaktive kvinner og menn i 20-årene. Gjennomsnittskondisjonen synker med ca. 6 % for hvert tiår eldre man blir. Og Kondiskalkulatoren vår er velgnet til å beregne om en person har lav eller høy fysisk kapasitet.

Alle resultatene kommer fra forskningen til Stian Aspenes, som fullførte doktorgraden sin ved CERG i 2011. Forskningen viste dessuten også at sannsynligheten for å ha opphopning av risikofaktorer for hjerte- og karsykdom øker jo dårligere kondisjon man har. For hver nedgang i kondisjonstall på 5 – for eksempel fra 50 til 45 – er sannsynligheten for å ha magefedme i tillegg til minst to andre risikofaktorer 56 % høyere.

Intervalltrening forbedrer blodårefunksjonen mer

Én treningsøkt gir akutte forbedringer i funksjonen til blodårene og reduserer blodsukkeret til personer med metabolsk syndrom, spesielt hvis intensiteten er høy. Metabolsk syndrom er en opphopning av minst tre risikofaktorer for hjerte-karsykdom.

28 pasienter med metabolsk syndrom ble fordelt tilfeldig til 4x4-intervalltrening, kontinuerlig trening med moderat intensitet, eller ingen trening. Vi brukte ultralyd til å måle blodårefunksjonen, og i starten av treningsprogrammet var den akutte effekten av trening større og varte lenger etter intervalltrening enn etter moderat trening. Det samme gjaldt blodsukkerresponsen. Etter at deltakerne hadde trent i 16 uker hadde derimot ikke treningsintensiteten like stor betydning for den akutte treningsresponsen.
